    About our Founder, Eloise Tweeten
    Eloise was born and raised in Madison , Wisconsin . As the daughter of a family physician and granddaughter of a Lutheran minister, strong personal ethics and the value of service were instilled in her from a young age. Her background includes a bachelors degree in psychology, masters degree in business, and over 25 years of administrative, management and consulting experience in human service professions.

    She was called to do this work with elders and their families based on first hand knowledge of the difficult and often sudden decisions families face when independent living for a loved one is no longer an option. Currently, both her father (age 92) and aunt (age 94) reside in assisted living facilities due to sudden changes in their physical health. She has been the primary care coordinator for her aunt Ruth since 2007, when Ruth lost her independence after a fall. Because Ruth lives in Tucson, AZ, Eloise also intimately understands the challenge of finding an appropriate assisted care home and managing a loved one's care from long distance. This experience inspired Eloise to launch TWEETEN ELDERCARE ADVISORS. Her associates, Sara and Don, are also Baby Boomers who have been through similar experiences with their own family members and share her commitment to service.

    What we do
    We specialize in matching you or your loved one with the best licensed assisted living home (Residential Care Facility for the Elderly) in Sonoma County based on your needs. With over 140 RCFEs to choose from, this could be a daunting task if you were to undertake it on your own! We provide this service free of charge to you.
    How we do it
    Assuming you are the person in need of assisted living, we start the process by gathering information from you, your family, your case manager, and/or anyone else whose input is important to help find the best match. If you live locally, we will also meet with you in person.
    We then go to work identifying three to five facilities that best match your criteria; support you throughout the decision making process; facilitate the transition; and check in with you after you’ve settled into your new home.
    The homes we place in
    • Are licensed Residential Care Facilities for the Elderly (RCFE) by the State of California
    • Have been personally visited by us and are regularly monitored
    Finding the Best Fit
    We believe both people and assisted living homes have unique personalities and needs. We are committed to finding a home that is well equipped to take care of your special needs, and would also be a good fit for both you and the home.
