The table of contents is omitted when you receive a "mobile" digest. You will get a mobile digest if you last opened a digest email on a mobile device.
Mobile digest omits the table of contents because many mobile email readers (such as the one on iPhones) do not support the linking technique that we use to jump between the table of the contents and where the full post is displayed lower down in the digest. Since that make that table content not function, we have removed it. Mobile digests also display squarish banners from our sponsors rather that wide ones that would be hard to read on a mobile device.
You can set a preference to always get a desktop formatted digest (with the table of contents) or a mobile digest by going here: https://www.waccobb.net/forums/profile.php?do=editoptions
