Looking for vet/vet tech with dog wheel chair experience

We have a good vet, but I need someone who has experience with dog wheelchairs/carts. He is losing strength in his back legs, but also has bad front elbow arthritis so I can't just order him standard wheels. Looks like they have good places in san mateo and Menlo park, but that drive would be too much for him.
